SINGAPORE - The wrong identification of a patient - resulting in the extraction of two healthy teeth - has led to the development of a more robust governance framework by the National Dental Centre (NDC), following a full review of internal safety processes. The incident, which took place at the NDC four years ago, had been traced to the attachment of the referral slip for extraction to the wrong patient call number chit, resulting in the wrong patient being called into the room for extraction. The staff had also failed to verify the patient's identity and the treatment needed. The dental surgeon in that case, Dr Debbie Hong Pooi Mun, 30, was fined S$3,000 after she pleaded guilty at a disciplinary inquiry by the Singapore Dental Council.
