SINGAPORE: Health Minister Khaw Boon Wan said hospitals in Singapore are making a crucial strategic change in emphasis. They are shifting focus from being disease-centric to wellness-centric. Writing in his blog, Mr Khaw said this shift requires hospitals to work closely with the patients' GPs and their families, and with other healthcare providers in the community. An example of this is the Changi General Hospital's Disease Management Unit and its first Community Health Centre at Tampines. Four hundred of its discharged diabetic patients are now being cared for through this public-private framework and every discharged patient has a personal nurse in virtual contact to ensure that the chronic disease is well-managed.
